All incoming student-athletes are required to cleared through our Athletic Training office before they will be permitted to tryout for a team.
Student-Athletes must have an up to date physical with the Athletic Training office. They can either have one completed by thier personal doctor or by the FLCC Physician on the listed dates. When completing a physcial, the student-athlete must document it with the following form. Athletic Physcial Form. Previous physicals are valid for one year after the date the physical exam was completed, however, if it expires during a season, the student-athlete will not be allowed to participate in games or practices as per NJCAA guidelines.
Student-Athletes will also be required to fill out a Pre-Participation Medical History form. This is only for incoming student-athletes and those who are new to the athletic program.
Many of these opening meetings coincide with the first tryout date, so it is recommended that you turn your paperwork in prior to competition. It can be either dropped off to the Athletic Training office at FLCC or faxed in to: 585-785-1439. Delaying submitting your information could result in delayed clearance to participate in your desired sport if the Athletic Trainer finds missing information.
